Important Emergency Information: Life threatening conditions, Allergies, Asthma, Concussion

Life Threatening/Allergy/Anaphylaxis/Asthma:  Forms are mailed home to all students during the summer.  They (along with photos) are kept in the main office in a binder as well as a copy sent to the bus companies and STEO.

If your son / daughter has a life threatening condition, allergies that require an epipen, or asthma, and we do not currently have the information on file, please print and submit the appropriate form as quickly as possible:

Concussions:  Completed forms are all kept in the binder in the main office.   Doctor forms are to be returned to the school following appointment if required.  If a staff member suspects a possible concussion, they will complete this form:

Parents / guardians are asked to have the doctor who assesses the student to complete the following:

If your son / daughter experiences a concussion outside of the school, please be sure to contact the office so that the school can follow the same protocol.
